Enrollment Process for 

Therapeutic Supervised Parenting Time 

 

DO NOT complete this form if you are trying

to register for a parenting course.

Please do not have another person complete this form on your behalf. 

 

1. Complete the Intake Form below. 

 

2. Pay the Intake Fee:  $75. Each party is responsible for their own intake fee.

3. Once we receive the form and the intake fee, we will reach out by email to schedule your intake appintment.

4. Gather your documents-Valid ID, Court Order, PPO, CPS Reports, etc. It is essential to have the current court order OR written agreement regarding supervised parenting time prior to the intake appointment. We cannot have an intake appointment without the order or agreement. 

 

5. Attend your IN PERSON intake appointment. We will discuss the family background and our policies and procedures. Please do not bring children or other 3rd parties to this appointment or it will be rescheduled.  The children will have a separate intake appointment to be scheduled.
