Enrollment Process for

Virtual Supervised Parenting Time

DO NOT complete this form if you are trying to register for a parenting course or for any kind of supervised parenting time that is not virtual.

Please do not have anyone complete this form on your behalf.

1. Complete the Intake Form below.

2. Prepay the Intake Fee of $40 (mandatory for each party to pay their own)

3. Once we receive the form and the intake fee, we will reach out within 24 business hours by email to schedule your intake appintment.

4. Gather your documents-valid ID, Court Order, PPO, CPS Reports, etc. It is essential to have the current court order OR written agreement regarding supervised parenting time prior to the intake appointment. Please submit these documents via email at downriverfamilyconnections@gmail.com.

5. Attend your virtual intake appointment. We will discuss the family background, and our policies and procedures, and scheduling.  Please do not have the children or 3rd parties present.
